Renovar Family Health Plus y Medicaid Managed Care
Si usted es actualmente enrolado en Family Health Plus o Medicaid Managed Care, usted recibirá una carta de su Departamento de Servicios Sociales (DSS) local del condado donde usted viva que dice que es hora para que usted renueve su elegibilidad para estos programas. Usted debe completar el formulario que se incluyó con su carta y devuélvalo, junto con ciertos documentos, por la fecha mostrada en su carta. Si el Departamento de Servicios Sociales local no recibe la información por la fecha dada en su carta, su cobertura acabará.
