How to Keep Your Coverage

We think everyone should have access to excellent health care services. That’s why, as a member of Hudson Health Plan, we think it’s important for you to take the proper steps to stay covered!

How and when you renew your coverage depends on your current status as a member. We can help you determine your current eligibility for either Medicaid Managed Care, or Child Health Plus, or Family Health Plus.

We’ll guide you through the process of renewal in your current program or help you enroll in the program that meets your needs today. Renewal in any of these programs is an annual requirement. Watch your mail for a letter and a renewal form. If you have any questions – contact Customer Care or call us at 1.800.339.4557.

Renewing Child Health Plus

If it’s time for your child to renew for Child Health Plus you will receive a packet in the mail from Hudson Health Plan. The packet contains a form which you must fill out and return to us in the supplied envelope. If you’ve received your packet and need help filling it out, please call us at 1.800.339.4557 or contact Customer Care .

Renewing Family Health Plus and Medicaid Managed Care

If you are currently enrolled in Family Health Plus or Medicaid Managed Care, you will receive a letter from your local County Department of Social Services telling you it is time for you to renew your eligibility for these programs. You must complete the paperwork that is enclosed with your letter and return it, along with certain documents, by the date shown on your letter. Be sure to include proof of your current income and any other changes that might have occurred since you last applied or renewed your coverage. The package contains a "Documentation Checklist" which shows you what documents can be used to document changes. If the local district office does not receive the information by the date given in your letter, your coverage will end.

For your convenience, we have providea list of County Department of Social Services offices  in our coverage area.  Be sure to bring your paperwork with you, including the documentation. Please remember that it is still your responsibility to return the paperwork to the local County Department of Social Services before the deadline, or your coverage will end.
