Renewing Family Health Plus and Medicaid Managed Care
If you are currently enrolled in Family Health Plus or Medicaid Managed Care, you will receive a letter from your local County Department of Social Services telling you it is time for you to renew your eligibility for these programs. You must complete the paperwork that is enclosed with your letter and return it, along with certain documents, by the date shown on your letter. If the local district office does not receive the information by the date given in your letter, your coverage will end.
If you need help, call us at 1.800.339.4557 or contact Customer Care. You can also call or visit your local County Department of Social Services office. For your convenience, we have provided a list of local County Departments of Social Services. Please remember that it is still your responsibility to return the paperwork to the local County Department of Social Services office before the deadline or your coverage will end.
Hours of operation vary from County to County, so it is advised that you call ahead before visiting.
