We offer three New York State-sponsored health care coverage programs that provide complete medical, dental, vision, and other health care services to our members:  Medicaid Managed CareChild Health Plus and Family Health Plus. If you are a member of Hudson Health Plan, then you belong to one of these three programs.

Each program is a little different. If you are not sure which health care coverage program you belong to, refer to your Hudson Health Plan ID Card.

Customer Care is here to help

Need help understanding how our program works? There is someone to help you at Customer Care  Monday through Friday, between 8:30 am and 5:00 pm at 1.800.339.4557 (TTY 1.800.855.2881). If you need help at other times, call 1.800.339.4557 for after-hours assistance.

You can call to get help anytime you have a question.

You may want to choose or change your Primary Care Professional (PCP), ask about benefits and services, get help with referrals, replace a lost ID card, report a pregnancy or the birth of a new baby, or ask about any change that might affect your benefits (for example, you get a job that offers health care coverage).

We offer free sessions to explain our health plan and how we can best help you. It’s a great time for you to ask questions and meet other members. If you’d like to come to one of the sessions, call us to find a time and place that are best for you.

If you do not speak English, we can help.

We want you to know how to use Hudson Health Plan, no matter what language you speak. Call us and we will find a way to talk to you in your own language. We have a group of people who can help. We will also help you find a Primary Care Professional (PCP) who speaks your language, if one is available.

For people with disabilities:

If you are in a wheelchair, are blind, or have trouble hearing, call us if you need extra help. We can tell you if a particular provider’s office is wheelchair accessible or is equipped with communications devices. Also, we offer helpful services including:

  • Relay link to TTY/ TDD machine (1.800.855.2881)
  • Information in large print
  • Case management
  • Help in making or getting to appointments
  • Names and addresses of providers who specialize in serving persons with disabilities.

Your Health Plan Identification Card

After you enroll, we’ll send you a welcome letter and your Hudson Health Plan ID card within 14 days of your enrollment date. Your card has your Primary Care Professional’s (PCP) name and phone number, and your client identification number on it. If it’s wrong, call us right away. Carry your ID card at all times and show it each time you go for care.
