Supporting Excellence
Supporting Excellence is a philosophy and an attitude that underlies all of Hudson Health Plan’s dealings with health care professionals and members. We believe that health care professionals share our goal of providing excellent health care services and we are committed to supporting and encouraging their efforts. We have organized our internal operations so that the information we send out is consistent, coherent, and actionable; so that all of our staff are well trained and well informed; and, so that we can systematically collect, assimilate and respond to feed-back from health care professionals and members. Supporting Excellence encompasses all the Case Management programs and projects we have to educate members about their health care needs, including the special services of our nurse case managers. It also helps us determine the best information to include in our quarterly member newsletter.
Since 1999 Hudson Health Plan has paid a bonus to primary care professionals based on the annual New York State Department of Health Quality Assurance Reporting Requirements report (QARR). Several years ago we created an additional immunization bonus program and in the last few years we have tested and developed a bonus program for the management of adult diabetes. We recently grouped these pay-for-performance programs with our disease management and patient case management programs under Supporting Excellence.
A component of Supporting Excellence is targeted at rewarding health care professionals for quality care. Hudson Health Plan understands that managing information can lead to better health outcomes for our members. We also know that this takes time and effort. To acknowledge this effort, Hudson Health Plan has programs to compensate health care professionals:
New York State QARR Measures (Quality Assurance Reporting Requirements)
- May exceed $1.25 per member, per month
- Reported and paid annually to Primary Care Professional
Well Women Care
- $50 for obtaining PAP specimen
- Reported and paid following visit
Special Care
- $50 for each completed medical assessment form returned annually
- Paid following submission
Pediatric Immunization and Screening
- Up to $200 per two-year old child
- Reported and paid quarterly to Primary Care Professional
Obstetrical Care
- Pre-natal registration $100
- Paid following registration call
- Postpartum visit $100
- Reported and paid to Obstetrical Clinician following timely visit
Diabetes Management
- Up to $300 per patient
- Reported and paid annually to Primary Care Professional
