Hudson Health Plan’s mission is “to promote and provide access to excellent health services for all people.” Because we believe that everyone has the right to be healthy and have access to the services necessary to stay healthy, we make it as easy as possible for anyone living in Westchester, Rockland, Orange, Sullivan, Ulster, or Dutchess Counties to determine their eligibility for the programs we offer.

Just follow these easy steps:

Step 1: The first step in enrolling with Hudson Health Plan is to start the application process online. The online application will help you see if you or your children qualify for one of our three programs: Medicaid Managed Care, Child Health Plus, or Family Health Plus. To do this, we will ask a few questions about the following:

  • Family size
  • Income
  • Age of family member(s)
  • Residency in Westchester, Rockland, Orange, Sullivan, Ulster, or Dutchess counties
  • In some cases, student or parental status

Step 2: The second step is to schedule a meeting with one of our Outreach Representatives.

Step 3: The third step is to come in to one of our local offices and meet a Outreach Representative. Our friendly staff can help determine which of our three programs—Medicaid Managed Care, Child Health Plus, and Family Health Plus—is right for you. We'll guide you through the enrollment process and help you identify the supporting documents needed to verify your eligibility. Please Note: Supporting documents are an important part of the application process. These may include a birth certificate and pay stubs or tax documents. For more information about which documents are necessary for your application, click on our document checklist in English or in Spanish.
